Caroline Gan Health Coaching provides a health coaching consultancy service with the aim of improving health and wellbeing outcomes for individuals through nutrition and other lifestyle interventions. This service is delivered through personalised coaching programmes based on diet and lifestyle analysis. Coaching sessions may take place online or in person. Depending on your personal circumstances you may be asked to seek the support of other therapists or additional medical support, and you will be advised of this at the earliest opportunity.

Your privacy is very important and I am committed to protecting your personal data. I will only ever use your personal information to enable me to provide the service that I offer, or to inform you of other similar services.

 

What information do you collect?

The information I collect will include basic details such as your name, address, contact details and next of kin.

I will also collect health information including your medical history, dietary, lifestyle, supplement and medicine details, clinic notes and health improvement plans.

I will also require your GP contact information and your bank details for payment purposes.

Why is this information collected?

I use this information in order to provide you with appropriate nutrition and lifestyle coaching to support you in improving your health and wellbeing. This information is also used to process payments.

How is my information collected?

You provide me with personal data in the following ways:

  • By completing a medical history and lifestyle questionnaire.

  • By signing a terms of engagement form.

  • During a nutritional and lifestyle consultation.

  • Through email, online via the website, over the telephone, via Zoom/Skype or by post.

  • When processing payments.

What is done with my information?

I use your information to provide direct healthcare to you, in the context of health coaching sessions.

I also use this information in the processing of payments.

I may use your personal data where there is an overriding public interest in using the information e.g. in order to safeguard an individual, or to prevent a serious crime.  Also, where there is a legal requirement such as a formal court order. 

I may use your data for marketing purposes such as newsletters or testimonials, but this would be subject to you giving us your express consent.

Is my information shared with anyone?

I will keep information about you confidential.  It will only be disclosed with other third parties with your express consent, with the exception of the following categories of third parties:

  • Our registrant body and our professional association (UKHCA)

  • Any contractors and advisors that provide a service to us or act as our agents on the understanding that they keep the information confidential.

  • Anyone to whom we may transfer our rights and duties under any agreement we have with you.

  • Any legal or crime prevention agencies and/or to satisfy any regulatory request if we have a duty to do so or if the law allows us to do so.

I will seek your express consent before sharing your information with your GP or other healthcare providers. However, if we believe that your life is in danger then we may pass your information onto an appropriate authority (such as the police, social services in the case of a child or vulnerable adult, or GP in case of self-harm) using the legal basis of vital interests.

I may share your case history in an anonymised form with our peers for the purpose of professional development.  This may be at clinical supervision meetings, conferences, online forums, and through publishing in medical journals, trade magazines or online professional sites.  We will seek your explicit consent before processing your data in this way.

How long is my information kept for?

All records held by Caroline Gan at Caroline Gan Health Coaching will be kept for the duration specified by guidance from our professional association UKHCA. This is currently 7 years. 

Where is my information kept?

Your information will be held securely online in a password protected folder.
